The Train Route — Fly to Cusco, acclimatise for 2 nights, take Peru Rail to Aguas Calientes. Morning buses take you up to the citadel.

This is non-negotiable. Cusco sits at 3,400m. Spend at least 2 full days in Cusco before doing any significant elevation gain. Drink coca tea, eat light, stay hydrated.
